Your Right to Receive a Good Faith Estimate
LifeMed Institute is committed to providing clear and transparent information about the expected cost of your health care services.
Under the No Surprises Act, individuals who do not have health insurance or who choose not to use their health insurance to pay for health care services (self-pay) generally have the right to receive a Good Faith Estimate of the expected charges for scheduled health care items and services.
Your Right to a Good Faith Estimate
You have the right to receive a Good Faith Estimate of expected charges when you schedule health care services or when you request an estimate from LifeMed Institute.
If you are uninsured or choose not to use your health insurance, LifeMed Institute will provide a Good Faith Estimate in accordance with applicable federal requirements.
The Good Faith Estimate will include expected charges for the health care items and services that LifeMed Institute anticipates providing as part of your scheduled care.
A Good Faith Estimate is not a bill and is an estimate of expected charges. Your actual charges may differ if additional services or items become medically necessary or are requested that were not reasonably anticipated at the time the estimate was prepared.
When You Should Receive Your Estimate
If you schedule care in advance, the timing for receiving your Good Faith Estimate depends on how far in advance your services are scheduled. You may also request a Good Faith Estimate before scheduling your care.

If Your Bill Is Higher Than Your Good Faith Estimate
If you are uninsured or self-pay and your final bill is at least $400 more than the Good Faith Estimate provided to you by LifeMed Institute for that provider or facility, you may have the right to dispute the bill through the federal Patient-Provider Dispute Resolution process.
You should keep a copy of your Good Faith Estimate and compare it with your final bill.
